Office Policies

The following information will give you a better understanding of our office policies. These policies are in place to help us provide quality medical care in a professional and timely fashion.

New Patients

We ask that you please set up a new patient visit as soon as possible to allow our providers to review your medical history and perform a physical exam. Please try to have your old patient records, especially the immunizations, sent to our office prior to this visit. Our new patient paper work can be found in our Patient Forms section.

Newborns

Please call our office upon discharge from the hospital to schedule your newborn appointment with the doctor. Again, the new patient paperwork is available for download from the website.

Refill Request

Routine refills of your medications should be handled during regular business hours so your doctor can review your pertinent medical information. At the time of your call, please let us know which medication you are requesting, the dosage, the number of doses per day and the pharmacy you prefer.

Requests placed at the end of the day may not be completed until the next day.

Please note that for ADHD medications we request 24 hours prior notice before being sent to the pharmacy.

Appointment Times

We ask that you please be on time for you appointments. We understand that unforeseen circumstances can occur and would appreciate a phone call if you know you will be late. Please note that if you arrive more than 15-20 minutes after your scheduled appointment you may be asked to reschedule.

We ask that new patients arrive 15 minutes prior to their appointment time to fill out new patient paperwork.
